Giant adrenal pseudocyst.
Adrenal cysts are rare lesions which are usually nonfunctional and asymptomatic. Adrenal pseudocysts do not have epithelial linings and account for about 40% of adrenal cysts. We report a case of giant adrenal pseudocyst in a 32 year old woman presenting with flank pain who had undergone surgical excision.

Cysts of the adrenal gland are uncommon. We report a case of symptomatic pseudocyst of adrenal gland in an adult male, which was excised through retropertoneoscopic route. Surgery was uneventful and patient returned to pre-operative activity within seven days of operation. At ten months of follow up patient is asymptomatic and doing well.

We describe the appearance of an adrenal pseudocyst on MRI and CT. The MR characteristics of the lesion were noteworthy in that the lesion had two components with different imaging characteristics. The larger component was of low signal intensity on both T1- and T2-weighted images and might have been confused with an adrenal adenoma.

With computed tomography (CT) and ultrasonography, more cysts are being detected incidentally. In one study, adrenal cysts were discovered incidentally in 0.02% of 12 000 CT scans. We describe a case of a giant adrenal pseudocyst mimicking a malignant lesion.
